Create and Submit a Self-Correct

Instructions:

  1.  Review and correct the violations listed in the Fire Inspection Report. 
  2.  For violation(s) corrected, initial and date next to each.

  3.  Print, sign, and date the bottom of the inspection report to confirm that all violations have been corrected.

  4.  Once the violations have been corrected and the report has been signed, you are ready to submit the “Self-Correct Affidavit” to the HBFD using the Accela Citizen’s Access (ACA) portal.
  5.  Log into your Accela Citizen’s Access account. If you have not registered for an account, then please review the “Create a New Account” guide.
  6.  In order to upload the self-correct affidavit, you must 1st become associated with the record.  Review the “How to Become Associated with a Record” guide. Features such as paying fees, reviewing inspection results and submitting self-corrects become available once becoming associated to the record.
  7.  On the home page, enter the “Record ID” into the global search area located at the top right had corner of the page. Include the FEO or FMF prefix that accompanies the six digit number.  Example: FEO-023237

  1.  Select “Attachments” from the “Record Info” drop down menu.

  2.  Choose the “add” button.

  3.  Click "add" again.

  4.  Upload the Self-Correct Report.  Following the naming convention below.
  1.  Once the file has been uploaded, click "continue”.

  2.  Type “Self Correct Affidavit” in the description box and click save.

  3.  When the attachment has been successfully uploaded, the green message will appear and you have uploaded the report.  The HBFD will be notified that a document has been uploaded to the record.
